Default fading brakes

I've got a brake problem that I need help with. My brakes work great slowing down from high speeds, and typical in town driving. But if I pump the brakes at all or make a few applications in a short amount of time, I get bad brake fade to the point that I have to stand on the peddle to stop. Its like the power disappears.
I don't have any warning lights, I've got fairly new pads and rotors (about 1000 miles), I have changed the fluid with new Castrol LMA, and have bled and re-bled the system using the procedures in the RAVE, but still have the problem.
Any help would be appreciated.

How many miles on your truck, how much brake material do you have left? You may have a master cylinder starting to fail or possibly the rubber brake lines are starting to swell up under multiple stops.

60K on the truck, i've got fairly new ss braided brake lines. No leaks at any of the wheel cylinders. The pads have worn down maybe 10% at the most. It seems like this behaves just the opposite of a typical master cylinder problem. The more I pump the brakes the worse it gets, if I give the peddle a rest for a while, the brakes come back 100%

I've got the same problem!

In literally stop and go, bumper to bumper traffic, I'm scared that I'm going to hit the person in front of me because the brakes seem to "fade" or I get a really hard pedal.

No offense intended I'm just trying to get some direction towards a fix. I'm leaning towards the brake servo being the problem, since that supplies power to the brakes. After that

OK, I'm posting what fixed this for future searches.
I found the vacuum line for the brake servo was lose at the end that connects to the intake manifold. There's a hokey plastic fitting that fits into the manifold along with a plastic vacuum line to the servo that didn't seem to keep a tight fit, so I replaced it with a metal fitting and good old rubber hose. The hose is a little bigger than the original line, so supply shouldn't be a problem. I can pump my brakes now and the vacuum power keeps up to the demand. No more hard peddle
