When do you hear the noise, hot/cold or idle or while driving?

the noise is more apparent when the engine is warm/hot, and it is most noticable at idle and at very low speeds...any ideas?

Think CATS. Sounds like you did something to up set the cats and killed one or both.
Before you replace them, find out what fired up the cats and turned it into a bag of marbles, like wires. plugs, fouled injectors, heagasket, that sort of stuff.

Crawl under the truck while it is running and rattling and see where it is coming from.
