2002 LR Disco 2 - hesitating ....help !

Hi. I have a 2002 LR disco 2 - 4.0 petrol.
Was running very rough and had 15 codes that had come up.
I changed out: all 4- O2 sensors, tps, all spark wires, new plugs, MAF, Coil and the two catalytic converters. (cats melted right out)
After this I poured water over the intake and found there was either a leak in the lower intake plenum or the lower injector rings were not sealed allowing a vacuum leak …sucked in water and bogged down. Thus causing lean codes and burning up the Cats...
After this I replaced all the intake gaskets and put new rings on my injectors...the lower injector rings were definitely loose and old.
Now, all the codes are gone except for P0141! This will only come up sometimes , not all the time.
It idles 100% better but under load going up a hill I am still feeling a slight hesitation and surging.
Its not stalling but definitely hesitating and seems to be under power.
Head gaskets were done one year ago.
Was reading up on the Crankshaft Position Sensor...Would this throw a code and have these symptoms?
Any help would be appreciated. I'm running out of ideas...

Clear the codes, run it up again and check again for codes.
Post the exact codes you get.

- 2001 Discovery 2 SE (NAS LHD) “MYRTLE”, 110K miles, 4.0L V8, NGK Iridium IX plugs, PCV mod, 180* T-stat, Hayden fan clutch/Dorman fan, removed TBH, SAI modded K&N filter, Safari Gard steel guards, Warn XD9000i winch, KO2 265/75R16 on steel rims, EBC rotors/Akebono pads, +2” TF lift & LEDs
- UltraGauge Blue, Foxwell NT510, Nanocom D2 V8 & TD5

I found this post I think it might help you,

Through a bit more digging I think that I just found the answer and solved the problem that was causing the SES light.

I found an explanation relating to P1415 and Land Rovers: "SAI runs when cold. Pump runs, blows air into the rail. Power is applied to the vacuum switching valve, which applies vaccum to the gulp valves so they open. You either have no fresh air or no vacuum to open the valves. The nipple at the reservoir behind the right wheelhouse gets oil-soaked and falls off. The vacuum valves fail as well."

So......I went outside just now and lookedthe pump and vaccum connections over. Turns out that the quick connector on my fresh air pump was partially disconnected. I pushed on the connector and it made a very satisfying "click" sound as it properly reconnected itself to the pump. Started up the engine and voila'........the SES light is gone!!!!!
