Ok so in following up and closing this thread (hopefully) I installed 4 new Bosch O2 sensors.

All the problems went away. The over-revving, O2 error codes, running rich, and ... the transmission oil temp light is off. I don't know if it's because I replaced the O2 sensors or sprayed the temp sensor connection at the transfer case with MAF cleaner ...and coated it with dielectric grease.... but all the problems are totally gone! The truck is responsive and NOT running rich anymore. I just got a huge jump in MPG.

I drove around thousands of miles running rich wasting massive amounts of gas .... and who knows what damage I did to the engine constantly blasting it from 2k RPM to 5K RPM .. but I didn't get any codes until recently so I didn't know anything was wrong. So if anyone is reading this and has de-catted (for offroad use only ahem cough cough) you know it could be your O2 sensors causing the rev to rocket and the car to run rich.

So the moral of the story is (I hope) make sure all 4 O2 sensors are Bosch and (preferably) installed at the same time. I was using Bosch in the front and WALKERS in the back. I think this happened over time .. from using WALKERS in the back ..

The Walkers stink .. in my opinion .. when all 4 were Walkers the performance was ok but not the same as before .. .when I removed the Walkers and installed Bosch in the front .. performance improved .. I just got lazy and left the Walkers on the rears because I didn't think the rear O2 sensors were good for anything ... as it turns out (it seems) the rears are very important and have an effect on the performance.

This is all from changing the rear O2 sensors.

The car almost feels like a new car now .. I am not getting 6-9 MPG anymore .. The performance just jumped up to par and so far (knock on wood) all performance issues have been solved.
