Most likely culprit is the hood sensor.. It is a common failure item.. I think it also might have a tip sensor in it.. not really sure, but that is where I would start.

The slipping out of drive issue seems most likely to be a linkage / cable or other hardware issue. Drop me a PM w/email and I'll send you some info on what to look at.
+1 on both, had both issues few years ago.
The hood switch is under the passenger side hood latch, very easy to replace.
My linkage just beside the transmission was corroded, needed some cleaning and lubrication. Better if you can replace but mine still works.
