I don't know about the codes, but I used to have a Toyota that did exactly the same thing at the pump, i.e. the nozzle becoming increasingly sensitive to overflow and needing a very low flow of gasoline.

As you suspected in this case, it was the breather hose that became more and more clogged so the dealer replaced it and that got rid of the problem. Never saw how it is set up on a Disco, so I don't know if it could be unclogged or if it is easier to replace it in your case.
